Rare Enemies have a 10% base chance of appearing in most Quests. This chance increases in Co-op Mode. All rare enemies have a chance of dropping

The element and difficulty of a Quest determine the variation in rare enemy spawn. Expert-difficulty spawns are dependent on the player's might value.
